Rev hammer targeting

Posted a topic similar to this on old forums, though it turns out the issue isn't as isolated as I initially thought. So after that big update a bit over a month ago, there have been occasional problems with displaying the basic hammer attack as out of range (and therefore registers no hit) because it doesn't properly focus on its target for some reason.

This is still especially prevalent during the ice wall phase for claw of jormag where it just won't hit anything regardless of how close I get (even with manual targeting).
