The second charge simply has no internal cooldown. So, when you have 3 charges, spending one puts the skill on a 1s internal cooldown. Then, when the counter says 2, if you use it, there's no icd, so double tapping the button will instantly spend both remaining charges and put the skill on a hard cooldown. Unless the game is thinking "you held this button for .00001 seconds too long, I am going to use the skill again" possibly caused by higher ping? Or rather an oversight. What I'm saying is that it's not something that occurs randomly for some weird reason, it's specifically because the skill lacks an internal cooldown that's causing this issue.
