This afternoon’s build corrected several bugs and issues, but we want you to know that we’re aware that not all issues were fully corrected with the build, including one blocking player progress in “Forearmed is Forewarned.” We will continue to work on the remaining issues and will post an update tomorrow morning to let you know the status of those efforts.
We sincerely apologize for the delay and thank you for your continued patience. Please know that we understand the impact this is having and we are doing what we can to resolve this quickly and get everyone into the episode. We’re looking forward as much as you are to this being resolved and everyone being able to play.
So is this fixed or not?
This is from the latest patchnotes:
Fixed an issue that blocked quest progress after players talked to the Primeval Queens or rendezvoused with Taimi and Blish at the portal in the "Forearmed Is Forewarned" chapter of the Living World episode "Long Live the Lich."
Im as confused as you are, Stephan says no. but notes says yes.. umm what? Need some clarification please.

The patch was for a Fractal server crash. Cant believe they considered that a priority over the LS crash. Should be all hands on deck for LS.
There are certainly two different teams, and the Fractal team got a task done - hence the patch. You can't deligate programmers of one team to content done by another team, because they wouldn't know what to do.
It seems all my chars are locked out. not just the one that is on the quest. so....This sucks. keep getting slapped down with a network error. can you guys do us a favor and tell us exactly what the hell went wrong once you got this fixed? because this is the first living world that i remember breaking like this.

Wait, tomorrow morning? You guys are calling it a day? Players/fans/loyalist have waited over 10 hours and you are waiting until tomorrow? Got it.
It's not the first time we've had to wait a day. Head of the Snake was delayed entirely for about 25 hours (A blizzard knocked out the studio's power). These things happen. Chill. Wait. Go back to other maps and grab some Mastery Points you don't have yet. Mount is gonna need a lot of them. Or do the Fractal. Or play another game. It's not like we pay a sub fee for GW2.
Aside from that, I think many players are unhappy about being unable to log in at all, and attempting to switch toons does not fix this issue.

A blizzard is a nature matter, this is humans fault, especially since the extra month this episode already had.

Good luck with the fix guys, I hope you get it sorted out soon so people stop kitten at you. I'm sure you are doing everything you can to get it back up as soon as possible.
Now to the tougher question, as someone who works in technology (previously as a Systems Admin/Engineer, now as a Developer), at which point should this issue be considered so production impacting that it should be rolled back, fixes created, and a new attempt at release be done later this week or next week?
I only ask because, in my experience, I would've been forced to roll back within an hour (maybe two) of a similar issue on our servers/applications.

To be fair, the programmers/network people have been hammering at this for just as long. They need rest and time to let things settle in their brains. Tired programmers trying to fix a bug are a good way to make it worse.
And I'm sure they're going to have a few people running some tests and checking a few things tonight, so they have more information to hand the fixers tomorrow morning. I may freely blame ANet for how bad this is going, but all the blame in the world doesn't give the fixers greater endurance.
every update the same... you guys are killing you own game with this useless updates. dont mark a launch date for the update if you dont even tested it kitten....
Still having issues, can't even go throught the first part (the trail) when the game crashes arguing conection problems, this time with the error code 58:11:5:535:101

I'm sorry, but right now, sorry isn't enough!
Seriously, the release was delayed to uphold quality standards? This is the standard? No one being able to progress?
Having the same issues we've had on PoF release! Maybe a reality check would be better, and realize that the Servers at Amazon are failing to meet up with the demands of new, popular releases.
After 4 months i finally convinced my brother and a few friends to come back to the game and get back. And what happened, not only were they discouraged by this, but since there's generalized instability in the servers, they got dcd halfway through a long instance of the previous story chapter, and all left.
So, i'm afraid sorry isn't any where near good enough!

please treat the devs as normal human beings. they're no machines. they still become tired, exhausted etc.. of course it is annoying not being able to play, but beep happens and the they will roll out a patch when the breaking bugs are fixed. (not the scarabs and roller beetles of course )
I feel like you guys should give everyone in the game with account access to the new story a 1 use item that unlocks a waypoint in the new map if possible. This is ridiculous.
I can't even get into any map on any of my chars before it gives me error messages and DC's me. W.T.F.
I expected a few little bugs and such but not this... I can't even waste time on other char's stories or collecting mats.

This is especially fun for me. When I drop, I get to play a coin toss drinking game of "Did my internet die again, or was the server tazed?"
The issues in game aren't the thing that bothers me most, it's the fact that it's become a regular thing with Living World releases. It's especially more unforgivable when you delay the release a few weeks from the normal cadence for "quality" standards. I REALLY hope that internally people are being held accountable and that there is a SERIOUS discussion on how to not let this happen again, because if not, you will be losing a lot of players not willing to put up with this.
Leave it to Anet to waste your one free day out of a 6 day work week with not knowing how to make something work... even after taking additional weeks to "assure the highest of quality"...
Anet also STILL hasn't managed to fix the 'switch to WIN10 Desktop', when you move to another zone. I was hoping this would have been finally addressed with this patch, but seems to be too much to handle.
The game, yes, has crashed many times. But everyone needs to keep a straight head. I know how much we all want to see what happens next, or just really want that new mount . ArenaNet is trying very hard to fix this issue. I know were all excited, but just get a nice warm cup of milk, go to sleep, and wake up tomorrow morning when the issue will most likely be fixed. Good luck players!

I can only think it is not really a bug (or a bug only), I think it is more low budget. They didn't (don't) have the money to ask for better servers from Amazon, so the servers we have now are insufficient to handle the amount of users trying to do the same thing. Who knows.
Just got on to do the Forewarned and Forearmed storyline, and this time I actually got to see the arm before being kicked. Not sure if this number set has been posted but the error number was 7:11:3:191:101.
The second instance after the last patch is working better, still crashing out with the same error but this time got ~5 minutes instead of 1minute. Hopefully there's enough chekpoints in the instance to allow it to be played to the end.

You're talking at Serious Gamers. Whenever a patch drops and anything goes wrong at all, they throw a screaming tantrum the entire time until they get distracted and run off after the shiny. They're people with no experience at game development and design who think they're experts, who think they're owed everything for spending almost nothing, and who have no idea how pretty much every MMOG is. SWTOR? Good luck, you might get to log in a day or two after a content patch - because that's how long it takes to patch. Anything from Blizzard? Try next week, they might have things working after the servers stop crashing. You name it, every MMO suffers it, every one of these people shriek about it every single time. Like clockwork, because they demand instant gratification.
guys if you actually read what Gail said:

Was able to do so before patch as well. I got a couple minutes into Taimi/Blish dialogue before crash and inability to login. It was right after the point where you use the invisibility thing around the Awakened guy.
