This still appears to be an issue. I just ran into this as well. Tried restarting the story step as well as restarting the game to no avail. Submitted a bug ticket but as this has been an issue for as long as it has been I'd be surprised if anything comes of it. -

Confirmed to be fixed, just completed it. -
That doesn't seem to be the case either, I did hit both dragons in the instance I was in. Also did quite a bit of damage to both champions and helped kill at least one of the crystals each time those events came up.

I had never done DS on the character I was on that did not get credit, so that does not seem to be the case for the Return To achievement at least.

Same thing happened to me. I was in a party with some of my guild mates and the party leader got credit but no one else did. Perhaps it's something to do with that? Some of the other comments in EotN map chat after we finished also lead to that same conclusion.
