(Bug) Personal Story missions buggy - NPC voices broken + other bugs

I recently reran the entire My Story, since I haven't done it in years, and the missions are very buggy.

 

In general, and this is true of the Season 2 missions as well, there's no sound for about one third of the NPC voice over lines. I see the chat box, but the voice sound just isn't there. This doesn't happen in the cutscenes, just when the named mission NPCs are talking. Usually it'll be something like: one NPC will say a line, and maybe another line, and then a second NPC will respond but their line is inaudible, then the first NPC will say something again (voice heard fine) then the second NPC will respond again, but this time I can hear their voice.

 

Some other specific mission bugs:

 

Chapter 8: Victory or Death - Temple of the Forgotten God - The door to the instance isn't where it's shown on the map.

Chapter 8: Victory or Death - Estate of Decay - The "closed door that had to be opened with explosives" was actually open.

Chapter 8: Victory or Death - A Grisly Shipment - In the final cutscene, the characters don't enter the Char Chopper, instead they slowly float up into the air after the Char Chopper flies away.

Chapter 8: Victory or Death - The Source of Orr - During the final battle to defend Treherne while he cleanses the Artesian Waters, the only sounds I could hear were the music and the NPC chatter. All sounds of fighting and everything else were just gone. They returned after the cut scene though.

Living World Season 2 - Tangled Paths - Tracking the Aspect Masters - Check on the Aspect Masters - after the Master of Wind says "I'm no good to our master like this. Please, see it though for me." The game client disconnects forcing me to redo the mission, which then disconnects again at the same place. I'm unable to complete this mission.

Anyway, I just played Heart of Thorns - 16. Hearts and Minds - Storm Mordremoth's inner sanctum - During the final battle with Mordremoth my character yells out "take to the air!" So I jumped up into an updraft. This updraft shot me upwards quickly and right out of the mission instance without any warning. So it was literally a trap that caused me to leave the mission and now I'll have to replay the entire mission from the beginning in order to defeat Mordremoth.

 

Please tell me that this is a bug and not some sadistic dev's idea of a "challenge."

i was in mission with peregrine.  all was going well, we had mordremoth down to about 50% or so, and i grab an updraft and go aloft.  the next thing i see is the announcement line "the instance owner has left"... and then i get kicked out.  we spent 40-50 minutes to get to that point... I feel seriously ripped off.

My bad.  I did not check the result from the 'Search'.

 

It seems, for some reason, the thread in question has been removed.  I've no idea why.  My apologies.

So the bug with NPC voices just randomly not being heard, happens not only in the Personal Story missions, but also in Season 2, Season 3 and Heart of Thorns missions.

 

I've gone through the file repair process as described on the support page. It didn't change anything.

 

I can't believe that the developers are ok with their story missions just being broke for years at a time.

Icebrood Saga > No Quarter > Distract, Destroy, Distrust

Rytlock follows you around in Drizzlewood Coast and does not fight. He just stands there watching while you fight people and attack armored cars. I can only assume that this is a bug. If he's present with you in the outside world why wouldn't he fight? If it wasn't intended for him to fight then why not just have him show up only in the instanced missions, just like NPC's do in the majority of game content?
