[Thunderhead Peaks Rifts don't spawn mobs]

Rifts get stuck at some % and stop spawning monsters, sometimes requiring to wait 10+ minutes for them to spawn. 2 of 3 locations has been the same.

Yes, I've killed 4 different purist champs and none of them counted towards achievement. A lot of people with same issue according to map chats. All stuck in Special Ops Advance Course 2. Hope they fix it

Can you be more specific please? the only escort i found lead to a kirin champion and not warden. do you remember the npc that starts it?
