Vindicator utility skills randomly switching places

Revenant utility skills are randomly swapping positions with each other upon legend swap. It happens with every type of activity from HP to events. This is very inconvenient and harmful, because you have to pay attention not only to the enemy, but also to the skiles, to see if they have moved. Unfortunately, being in a fight you can't rearrange it.

Why hasn't this issue been fixed? Or even addressed?

The utility skills actually swap the moment combat begins. Out of combat I can swap legendary stances and see no change on my utility bars. The moment I'm in combat, the first legendary stance swap will reset utility skill positions every time.
