Happened to me as well, twice now. Both at the end of Visions of the Past; starts EoD instead of No Quarter. It doesn't just start the Cinematic, but it starts the Chapter itself (Mail from Caithe, Aurene talking about wanting to see you again, NPC placements etc). Still bugged, report sent in-game.
