What on earth is going on with this sequence of events/achievements?

There are four events in the chain, the first one beginning with 

Gather fungal residue to stabilize Verasha, the Speaker defector

So far over three days I have never seen this event take place. Is it bugged?

It's blocking this achievement and also getting a part for the Unchained Specialised weapon, Crushing weight.

Anet is there any chance you can get some of your QC/Playtesters/Developers to take a look at this, and by taking a look I don't mean spawning the event chain with a developer command on your in house machines, but doing it on a live play server?

3rd day now looking for this event.

This morning logged in and saw the last part of the chain was active.

By the time I got there, I managed to hit the boss 2 times only and the collection did not progress.

I am trying to finish Character Growth.

Not sure it is bugged, at least not for me.  I've done it a few times; last time was 2 days ago.

Found another possible hint that the map is bugged.

Near the [&BEANAAA=] https://wiki.guildwars2.com/index.php?title=Temple_of_the_Dredge POI there is a small altar with two NPCs building it on the west side.

The NPCs are usually dead and fast spawning crazed mobs make sure those two are dead.

Tried once to keep them alive for ~10 minutes while they seems to work on that altar, but nothing happens.

I switched maps today looking for the 1st mission in the chain only to find the last mission in the chain active, like usual.

The alter in question was built and the two NPCs were not there.
