Problem with skin

I recently returned to the game and am extremely unfamiliar with the wardrobe. I read that equipping/scrapping items will add their skin to the wardrobe, but even after equipping the Holosmith sword you get for training holosmith, it still doesnt appear in the wardrobe. Is there something else I need to do?

Equipping an item does not automatically account- or soulbind every item (there are exceptions), which is a required procedure if you want it added to the Wardrobe.

So simply right-click the item and select "Add to Wardrobe" from the menu that pops up.

There is no add to wardrobe option on right click. There is an unlock skin option that is greyed out though.

If the wardrobe option is greyed out, then the skin is in your wardrobe.  I'm not sure how your looking for the skin though.  Equip a sword, go to the wardrobe tab on your hero screen and left click the item.  If your trying to reskin your sword, the sword skin should be available to you.

If an item is account bound (or soul bound) on acquire, like the specialisation weapons, then the skin is unlocked in the wardrobe when you first obtain it, not when you equip it.

You can always tell if the skin on an item is in the wardrobe because when you hover the cursor over it one of the lines in the info box will say 'skin unlocked' in white text 'skin locked' in grey text or 'transmuted' and then the name of the original item. (And if it's your own item that means you have to have the skin unlocked because it's the only way to transmute something.)

You can see all the skins you've unlocked for all item types by going to the bank and clicking on the 3rd tab on the left side - that shows you the wardrobe and all the unlocked skins you've got (and most of the locked ones you haven't got yet).

You can see skins for items you have equipped and change them by using the wardrobe tab in the hero panel: simply click on the item you want to change and available skins will appear in the list on the left. But I recently found out you also need at least 1 transmutation charge in your wallet to do this, otherwise the list of available skins appears blank.
