I often can't do any damage to a player because everything I cast has no effect.  E.g., I rolled away from a thief, then cast fire/earth signets and no conditions landed despite both going on cooldown.  I can't explain it.

Maybe you have been blinded? Thieves can blind you with pistol #5 for example.

Blind only works once.  There was no block/miss text.  Signets can't be reflected?  You can't cleanse 7 conditions instantly.
There was no sight obstruction.  Today I was testing signets on a mist sparring bot.  I found that some of the ele spells have a bad casting mechanic.  If you are facing more than 90 degrees away from the target while moving they fail with no message and go on like 3 second cd.  So basically you can't fight a melee class without stopping for every cast because they can stand on you and strafe through your character model to be untargetable.  It looked like I was facing the opponent with distance in the example case, so I don't know why it happened there.
